On your first visit, it is essential that you have a valid referral from your GP or other Specialist, and bring a detailed list of all current medications including dosages. If you have recent test results, or relevant medical notes, please bring these also.

A Patient Information Form must be completed and returned to our surgery, or bought to your appointment.
